Requests for criminal history record information may also be submitted to the DCI on the required forms, by mail or fax. We do not accept requests for criminal history record information over the phone or by email.
You may download the required forms from the Iowa Department of Public Safety website at www.dps.state.ia.us
The DCI can also mail, fax or email the required forms to you. Please contact our office at 515-725-6066 to request forms.
You may also visit our office during normal business hours and conduct a criminal history record check on yourself. You must provide a government issued photo I.D. (i.e. driver's license, passport, etc.). We are located at 215 E 7th Street, Des Moines Iowa. Our business hours are Monday – Friday, 8:00 a.m. – 4:30 p.m. Processing hours for walk-in record check requests, 8:00 a.m. – 4:00 p.m. Please note: we are only able to provide immediate results if you are the subject of the request, per Code of Iowa, Chapter 692.2 (1). If you are searching on someone else you will be required to leave the request with us for processing.
The processing time for requests that are mailed or faxed back varies depending on volume of requests and staffing levels. The average processing time in our office for requests mailed or faxed is 2-5 days, excluding mail time.
The fee for all mailed, faxed or requests made in person is $15.00 per last name.
